Job summary

BayCare Health System is seeking a compassionate and skilled Occupational Therapist with SNF experience to join our interdisciplinary rehab team at Morton Plant Rehabilitation Center in Clearwater, FL. The OT will help patients regain independence in ADLs and IADLs, working alongside PT and ST colleagues in a fast-paced setting.

Responsibilities include evaluations, treatment planning, ADL retraining, and discharge planning with CMS-compliant documentation.

Qualifications

• Florida license as Occupational Therapist (OTR) or eligible
• BLS (Basic Life Support) required
• CMS guidelines knowledge for SNF documentation
• Minimum 1 year experience in SNF or subacute rehab preferred

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans
• Deliver occupation-based interventions focusing on ADL retraining and functional mobility
• Implement preparatory activities such as therapeutic exercise and cognitive interventions
• Collaborate with other therapists, nurses, physicians, and case managers to ensure coordinated care
• Educate patients and caregivers on adaptive equipment, home safety, and discharge planning
• Document progress and outcomes according to CMS and facility guidelines
• Participate in interdisciplinary meetings and care planning
• Maintain productivity and ethical, patient-centered care

BayCare is seeking a compassionate and skilled Occupational Therapist with SNF experience to join our interdisciplinary team. The ideal candidate will provide high‑quality, evidence‑based occupational therapy services to patients recovering from neurological, orthopedic, cardiac, and complex medical conditions. You will play a key role in helping patients regain independence in activities of daily living (ADLs) and instrumental ADLs (IADLs).

Occupational Therapist Responsibilities:

• Conduct comprehensive evaluations and develop individualized treatment plans.
• Deliver occupation‑based interventions focused on ADL retraining, transfer training, and functional mobility.
• Implement preparatory activities such as therapeutic exercise, balance retraining, and cognitive interventions.
• Collaborate with physical therapists, speech therapists, nurses, physicians, and case managers to ensure coordinated care.
• Educate patients and caregivers on adaptive equipment, home safety, and discharge planning.
• Document patient progress and outcomes in accordance with CMS and facility guidelines.
• Participate in interdisciplinary team meetings and contribute to care planning.
• Maintain productivity standards while ensuring ethical and patient-centered care.

Qualifications

• Current Florida license as an Occupational Therapist (OTR). Or eligible to obtain.
• BLS (Basic Life Support) required.
• Bachelor's degree Occupational Therapy required
• Minimum of 1 year experience in a Skilled Nursing Facility (SNF) or subacute rehab setting preferred.
• Knowledge of CMS guidelines and SNF documentation requirements.
• Strong clinical reasoning and communication skills.
• Ability to work effectively in a fast‑paced, team‑oriented environment.
• Experience with neurological, orthopedic, and medically complex patients is a plus.
