Job summary

Oasis Therapy Services is seeking an Occupational Therapist (OT) to provide one-on-one rehabilitation services at an assisted living facility in Cannon Falls, Minnesota. This role is available as PRN.

The OT will lead resident evaluations, develop individualized intervention plans, and focus on ADLs, mobility, safety, and independence while collaborating with the care team and documenting in Net Health.

Qualifications

• Active Minnesota OT license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Ability to work independently and collaborate with residents, staff, and therapy professionals.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience preferred.

Responsibilities
