Job summary

Oasis Therapy Services in Cannon Falls, Minnesota, is seeking a Certified Occupational Therapy Assistant (COTA) to provide one-on-one rehabilitation services at an assisted living facility. This PRN position offers flexible scheduling and the chance to impact residents’ ADLs, mobility, safety, and independence.

The role requires an active Minnesota COTA license, NBCOT certification, CPR, and experience in geriatric care or similar settings. Net Health EMR is used for documentation.

Qualifications

• Active Minnesota Occupational Therapy Assistant license.
• NBCOT COTA certificate in good standing.
• Graduate of an accredited Occupational Therapy Assistant program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Ability to work independently while following Occupational Therapist supervision.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.

Responsibilities

• Provide occupational therapy interventions delegated by and under the supervision of a licensed Occupational Therapist.
• Carry out individualized intervention plans developed by the supervising Occupational Therapist, focusing on ADLs, mobility, safety, and independence.
• Monitor residents' progress and report changes, concerns, or the need for reevaluation to the supervising Occupational Therapist.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Complete accurate Net Health documentation and collaborate with the care team.
