Job summary

Oasis Therapy Services seeks a Certified Occupational Therapy Assistant (COTA) to provide one-on-one rehabilitation at an assisted living facility in Cannon Falls, Minnesota. PRN option and Net Health EMR are used.

responsibilities include implementing plans under an OT, monitoring progress, and educating residents and caregivers. Requirements include an active Minnesota OTA license, NBCOT COTA certification, CPR, and graduate of an accredited OTA program; geriatrics experience is preferred.

Qualifications

• Active Minnesota Occupational Therapy Assistant license.
• Active NBCOT COTA certification in good standing.
• Graduate of an accredited Occupational Therapy Assistant program.

Responsibilities

• Provide occupational therapy interventions delegated by and under the supervision of a licensed Occupational Therapist.
• Carry out individualized intervention plans focusing on ADLs, mobility, safety and independence.
• Monitor residents’ progress and report changes to supervising OT.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Complete accurate Net Health documentation and collaborate with the care team.

Responsibilities

• Provide occupational therapy interventions delegated by and under the supervision of a licensed Occupational Therapist.
• Carry out individualized intervention plans developed by the supervising Occupational Therapist, focusing on ADLs, mobility, safety, and independence.
• Monitor residents’ progress and report changes, concerns, or the need for reevaluation to the supervising Occupational Therapist.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Complete accurate Net Health documentation and collaborate with the care team.

Qualifications

• Active and current Minnesota Occupational Therapy Assistant license.
• Active NBCOT COTA certification in good standing.
• Graduate of an accredited Occupational Therapy Assistant program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Ability to work independently while following required Occupational Therapist supervision.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.
