Job summary

Oasis Therapy Services in Cannon Falls, Minnesota seeks an Occupational Therapist to provide one-on-one rehabilitation at an assisted living facility. This PRN role focuses on ADLs, mobility, safety, and independence for seniors.

The OT will evaluate and reevaluate residents, develop individualized intervention plans, document in Net Health, and collaborate with a care team. Minnesota OT license and CPR certification are required; geriatric experience is preferred.

Qualifications

• Active Minnesota Occupational Therapy license required.
• Graduate of an accredited Occupational Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Ability to work independently and collaborate with residents, staff, and therapy professionals.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience preferred.

Responsibilities

• Direct the resident evaluation and reevaluation process.
• Develop and document individualized occupational therapy intervention plans.
• Provide therapy focused on ADLs, mobility, safety, and independence.
• Monitor residents' progress and modify intervention plans as needed.
• Direct and document discharge planning and recommendations.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Complete Net Health documentation and collaborate with the care team.
