Job summary

Oasis Therapy Services in Apple Valley, Minnesota, seeks an Occupational Therapist (OT) for one-on-one rehabilitation with seniors. This is a part-time position with opportunities for flexible scheduling and occasional travel to Cannon Falls.

You will provide ADL-focused therapy, monitor progress, and document outcomes in Net Health. The role requires an active MN OT license, an accredited OT degree, CPR certification, and reliable transportation for occasional travel.

Qualifications

• Active Minnesota Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience preferred.
• Reliable transportation and willingness to travel to Cannon Falls when needed.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy focused on ADLs, mobility, safety, and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Travel to Cannon Falls or provide telehealth support there as needed.
• Complete accurate Net Health documentation and collaborate with the care team.
