• Able to pass background check, drug screen, professional references check & physical evaluation.

Responsibilities

• Care Needs & Planning: Assist to implement & revise care plans, monitors health conditions, supplements direct care needs & responds to emergencies or changes in status.
• Staff Oversight & Scheduling: Observes care assignments, manages coverage needs, evaluates staff performance, educate & lead staff.
• Family & Team Liaison: Communicates changes in resident conditions to families/providers, works collaboratively with the interdisciplinary team.
• Documentation: Reviews and investigates incidents, transcribes orders, audits medical charges and maintains record compliance for accuracy.
• Family, Resident, Staff Education: Explain health status, changes and expectations for care, guides and direct care skills with intervention as needed.

• 3-11pm Every Friday (8hrs/Wk)

• Long Term Care Experience Required
