Qualifications

• Licensed to work within the State of Connecticut as an LPN.
• Able to pass background check, drug screen, professional references, and physical evaluation.
• Professional and clinically skilled with leadership qualities.

Responsibilities

• Administer medications including IV and injections.
• Wound care and pain management; apply dressings.
• Monitor health status and analyze changes.
• Provide catheter/ostomy care and collect specimens.
• Execute individualized care plans.
• Collaborate with physicians, therapists, and social workers.
• Educate residents and families about health status and care expectations.
• Maintain detailed health records and care progress.
• Oversee CNAs to ensure quality care and teamwork.

Here’s What You’ll Do for Our Residents

• Administer Medications: Give oral, topical, IV, injectable medications
• Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings
• Monitoring: Observe physical and mental health status- track changes & analyze data
• Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.
• Care Planning: Assist in executing individualized care
• Collaboration: Strategize care with physicians, therapists, social workers & others
• Family & Resident Education: Explain health status, changes and expectations for care
• Documentation: Keep a detailed record of health status, all care, progress & interactions
• Team Oversight: Oversee & delegate CNAs ensuring quality care/teamwork
