Job summary

Willow Tree is seeking a compassionate Nursing Home Admissions Concierge to oversee the admissions process at our Willow Tree community. As the first point of contact for potential residents and families, you will provide a welcoming and informative experience while guiding transitions into care.

This role requires concierge-level customer service, proactive communication, and coordination with medical teams to determine care needs.

Qualifications

• Must have a valid driver’s license for travel to hospitals, referral partners, and community events.
• Active LPN or RN license to practice in Colorado, in good standing.
• Exceptional communication and interpersonal skills to engage with residents, families, and healthcare providers.
• Strong organizational abilities to manage multiple admissions at once.
• Knowledge of healthcare regulations and admissions procedures in a nursing home setting.
• Sales and marketing experience preferred, with the ability to present the community’s services and benefits.
• Proficiency in computer systems for data entry and record-keeping.

Responsibilities

• Serve as the primary point of contact for prospective residents and families, providing information on services, amenities, and costs.
• Conduct initial consultations via phone, email, or in person to assess needs and eligibility.
• Schedule and lead engaging community tours, tailoring each visit to the unique needs of the prospective resident.
• Guide families through the admissions process with clear communication and compassionate support.
• Coordinate with hospitals and discharge planners to facilitate a smooth transition from hospital to community.
• Prepare admission paperwork and ensure compliance with regulatory requirements.
