Job Overview

We are conveniently located on the West Bank of the Mississippi River near downtown Minneapolis.

• Transitional Care Unit
• 28 beds
• Serves multiple diagnoses including neuro, ortho, oncology, amputee, spinal cord injury, transplant, etc.
• All adult patients
• Post-Acute – PT, OT, SLP.
• Approximately 50 therapy and tech staff at the 2 post-acute units
• Highly Collaborative between all fields along with MD/Physiatrist, rehab nursing, social work, dietary, etc. with primary goal to get patients home.

This position will be working 64 hours every two weeks on the evening shift, 8 hour shifts, and every other weekend. The position is eligible for benefits.

Responsibilities:

• Provides basic patient care by performing routine duties to include: Partners with patient care giver in care/decision making.
• Personal grooming, bathing, toileting, feeding, skin care needs (clean and sterile dressing changes), applying ointments/creams, oral hygiene, specimen collection/testing, position/ambulating, changing linens.
• Promptly answers call lights.
• Assists nurse in monitoring patient condition by accurately taking and recording weights, input/output, blood pressure, temperature, pulse rates, respiration, oxygen saturation.
• Assists with admissions, transfers, and discharge of patients.
• Orients patient/family to environment, assuring the patient’s belongings are accounted for, documented upon admission, transfer and discharge.
• Transports patients, equipment, specimen and supplies.
• Cleans, sets up, and maintains rooms and equipment.
• Orders and stocks supplies.
• Correctly documents tasks performed on appropriate form.

Required:

• Current on the MN CNA Registry
• BLS (basic life support) for healthcare providers

Preferred:

• Experience in a hospital setting
