Job Description: MDS Coordinator at Providence Place

Providence Place, a Lifespark Skilled Nursing Community, is seeking a dedicated and passionate MDS Coordinator to join our growing team.

Located in southern Minneapolis in the Standish Ericcson neighborhood, Providence Place provides services for seniors in a TCU (Transitional Care Unit), Long-Term Care, and Memory Care. We have been helping seniors Age Magnificently for over 25 years and are part of Senior Care Communities, a local non-profit supporting seniors.

Schedule Available: Full Time Days with occasional weekends
Wage: $44-$47 based on experience
Sign-on bonus: $2,500

This role involves overseeing the scheduling, completion, and submission of all necessary documentation (MDS) to ensure compliance and positive resident outcomes. Responsibilities include:

  1. Educating nursing staff on proper documentation for quality care, compliance, assessments, and interventions, including POC training for nursing assistants and PCC clinical assessments.
  2. Ensuring timely and accurate completion of the RAI process, including MDS, CAAs, and the resident's plan of care.
  3. Collaborating with the Interdisciplinary Care Team to develop comprehensive assessments and care plans.
  4. Verifying documentation accuracy and compliance with State and Federal guidelines.
  5. Determining Medicare/Managed Care eligibility for admissions and readmissions.
  6. Coordinating Medicare/Managed Care meetings and reviewing coverage status.
  7. Managing Medicare denial letters and coordinating the appeals process as needed.

Qualifications:

  1. Valid state Registered Nursing licensure.
  2. At least one year of experience in long-term care with a strong background in MDS and regulatory compliance preferred.
