Oak Street Health in Chicago, IL is seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, value-based primary care to adults on Medicare. Practitioners see patients independently within a collaborative care team in outpatient settings.

This full-time role requires MSN/DNP or NP certificate, an active NP license, DEA, and US work authorization. Competitive pay range ($95,738–$206,206) plus medical, dental, vision, PTO and retirement benefits.

Qualifications

• Master of Science in Nursing or higher in nursing
• National certification in NP specialty
• Active state NP license and DEA license

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer wellness visits and health risk assessments.
• Care coordination with providers and facilities.
• Population health leadership within the care team.
• Educate patients on conditions and care plans.
• Participate in promotional activities and home visits as needed.
