Job summary
Oak Street Health is seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, value-based primary care to Medicare beneficiaries in outpatient clinics. You will join a collaborative team including physicians, physician assistants, medical assistants, a dedicated scribe, RNs and care managers to support patients in staying healthy and out of the hospital.
Qualified candidates hold an MS/ DNP or NP certificate, an active state NP license, a DEA license, and US work
Qualifications
- MSN, DNP, or NP certificate required.
- Active NP license in the state.
- DEA license required.
- US work authorization.
- Board certification in chosen NP specialty.
Responsibilities
- Provide exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer annual wellness visits and HRAs.
- Care coordination with other providers and facilities.
- Population health leadership with the care team.
- Educate patients about health conditions and treatments.
- Participate in Oak Street Health promotional activities.
- Conduct home visits as needed.
