Job summary

Oak Street Health in the United States is hiring a Nurse Practitioner for our advanced practice provider role focused on value-based primary care for adults on Medicare. You will practice in outpatient clinics, collaborating with a physician-led care team to deliver comprehensive, patient-centered care.

Responsibilities include conducting visits, annual wellness checks, risk assessments, and coordinating with specialists. Home visits may be required.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program graduate.
• National certification in one of the following specialties: Family NP, Adult-Gerontology Primary Care NP, Adult NP, Gerontological NP.
• Active, non-probationary state Nurse Practitioner license; Active DEA license; US work authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
