Job summary

Oak Street Health in the United States is seeking a Nurse Practitioner, Advanced Practice Provider to deliver value-based primary care to Medicare adults in outpatient settings.

You will work with a collaborative care team to provide longitudinal care, perform wellness visits, coordinate with specialists, and contribute to population health initiatives within an innovative care model.

Qualifications

• MS in Nursing, DNP, or NP certificate program graduate.
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
• Active, non-probationary state Nurse Practitioner license in the US.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership with the Care Team (e.g., ensuring breast cancer screening every 2 years).
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions and care plans.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
