Qualifications

• MSN, DNP, or NP certificate required.
• National certification in one of: Family NP, Adult-Gerontology PCP NP, Adult NP, Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US work authorization.

Responsibilities

• Provide exceptional primary care and conduct office visits.
• Administer annual wellness visits and health risk assessments.
• Coordinate care with physicians, specialists, testing facilities, and agencies.
• Lead population health initiatives with the care team.
• Educate patients about conditions, plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties, as assigned.
