Oak Street Health seeks a Nurse Practitioner, Advanced Practice Provider to deliver effective value-based primary care to adults on Medicare, helping keep them happy, healthy, and out of the hospital.

Practitioners see patients independently and collaborate with physicians in outpatient clinics, focusing on primary care, care coordination, and population health. Anticipated weekly hours are 34; this is a full-time role with comprehensive benefits.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program graduate.
• Active, non-probationary state Nurse Practitioner license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Provide longitudinal care and build patient relationships for trust and adherence.
• Practice in outpatient setting on a collaborative care team.
• Coordinate care with other providers, specialists, testing facilities, and agencies.
