Job summary
Conviva Senior Primary Care, part of Humana’s leadership in value-based senior healthcare, is hiring for a Nurse Practitioner or Physician Assistant to join a multidisciplinary team at the Royal Palm Beach Center in Florida. You will deliver comprehensive primary care tailored to seniors and coordinate with physicians, pharmacists, and care coaches to optimize quality outcomes.
The role emphasizes collaboration, on-call duties, and growth within a supportive culture.
Qualifications
- Master's degree in nursing or PA program with board certification.
- Active, unrestricted NP or PA license in the state of practice.
- TB screening may be required.
Responsibilities
- Provide comprehensive care to seniors following clinical standards.
- Collaborate daily with the care team to ensure high-quality care.
- Manage referrals, hospital/SNF coordination, DME, and home health services.
- Document care accurately with support from quality-based coders.
- Participate in on-call rotation and growth plans at the center.
- Demonstrate passion for value-based care.
