Job summary

Humana Inc. through Conviva Senior Primary Care seeks a Nurse Practitioner or Physician Assistant to deliver comprehensive value-based care for seniors at the Wellington Family Center.

You’ll join a multidisciplinary team, manage referrals, coordinate hospital and home health services, and ensure accurate documentation while supporting on-call rotations. Required are an NP or PA degree, board certification, and an active state license with DEA credentials.

Qualifications

• Master’s in nursing or PA certification with board credentials.
• Active, unrestricted NP or PA license in the practice state.
• DEA license and Medicare/Medicaid numbers or ability to obtain.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate with care team to ensure high-quality, coordinated care.
• Manage referrals, hospital/SNF coordination, DME and home health services.
• Document care accurately with support from quality coders.
• Participate in on-call rotation and growth initiatives at the center.
