Job summary

Conviva Senior Primary Care, part of Humana's Primary Care Organization, is seeking a primary care physician to join our team full-time at CenterWell Primary Care in Brandon, Florida. The role is ideal for a compassionate clinician who thrives in a collaborative, patient-centered environment and improving outcomes for adult and geriatric patients.

You will deliver outpatient care, maintain records, diagnose and manage conditions, and work with interdisciplinary teams.

Qualifications

• MD or DO from an accredited medical school and board-certified or board-eligible in family medicine, internal medicine, med-peds, or geriatrics.
• Two years of experience managing chronic conditions.
• Active, unrestricted Florida medical license (or ability to obtain).
• DEA license or ability to obtain.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Collaborate with interdisciplinary teams to ensure holistic care.
