Job summary

Conviva Senior Primary Care (a Humana company) seeks a Nurse Practitioner or Physician Assistant to provide comprehensive, value‑based care to seniors at the Royal Palm Beach Center in Florida. You will join a multidisciplinary team and report to the Associate Medical Director, delivering high‑quality care and coordinating referrals, DME, hospital and home health services.

Qualifications

• Master's Degree in Nursing or PA program with board certification.
• Active, unrestricted NP/PA license in the state of practice.
• TB screening required for selected candidates.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Deliver value-based care.

Education
