This role is patient facing and part of Humana/Senior Bridge's TB screening program.

Responsibilities

Evaluates and treats center patients according to standards of care.
Monitors care using data and chart reviews.
Coordinates patient services including referrals and hospital/SNF coordination.
Leads with the care team through daily huddles.
Supports Regional Medical Director and Center Administrator to foster cooperation.
Completes medical record documentation with a quality-based coder.
Ensures compliance with licensing, certification, and accrediting bodies.
Participates in growth opportunities for new or existing services.
Engages in local primary care on-call program.
