Job summary

Conviva Senior Primary Care in Jacksonville, FL seeks a Nurse Practitioner or Physician Assistant to provide comprehensive, value‑based care for seniors at our on‑site clinic. You will collaborate with physicians, pharmacists, care coaches, and behavior health specialists to deliver personalized, high‑quality care.

You will report to the Associate Medical Director and will be integral to coordinated referrals, hospital/SNF coordination, DME, and home health services.

Qualifications

• Master’s degree in nursing with board certification.
• Active, unrestricted NP or PA license in Florida.
• Experience in value‑based care, primary care, or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain them.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high‑quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality‑based coders.
• Participate in on‑call rotation and support growth plans at the center.
• Work on‑site at the Jacksonville, FL clinic.
