Job summary

Conviva Senior Primary Care, part of Humana's Primary Care Organization, seeks a full-time Primary Care NP or PA to join our Edgewater, FL clinic. You’ll provide comprehensive, value-based care to seniors, collaborating with physicians, pharmacists, care coaches, and behavioral health specialists to deliver high-quality, patient-centered care.

You will report to the Associate Medical Director, manage referrals and hospital coordination, document care with coders, and participate in on-call

Qualifications

• Master's degree in nursing with board certification.
• Active, unrestricted NP, or PA license in the state of Florida.
• Experience in value-based care, primary care or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

• You will provide comprehensive care to seniors following clinical standards.
• You will collaborate daily with your care team to ensure coordinated, high-quality care.
• You will manage referrals, hospital/SNF coordination, DME, and home health services.
• You will document care with support from quality-based coders.
• You will participate in on-call rotation and support growth plans at your center.
• You will have experience with providing value-based care.
• You will work on-site.
