Job summary

Conviva Senior Primary Care, part of Humana’s Primary Care Organization, seeks a full-time NP or PA for our Ormond Beach, FL clinic. You will join a collaborative team delivering value-based care to seniors and coordinating with physicians, pharmacists, care coaches, and other specialists.

You will report to the Associate Medical Director and work on-site in a caring, inclusive environment with a strong growth focus.

Qualifications

• Must have a Master’s degree in nursing with board certification or PA program with board certification.
• Active, unrestricted Florida NP/PA license.
• At least one year of value-based care, primary care, or Medicare Advantage experience.
• Active DEA license and Medicare/Medicaid provider numbers, or ability to obtain.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support center growth plans.
• Work on-site.
