Job summary

Curana Health is seeking a Nurse Practitioner or Physician Assistant to lead primary care in senior living and skilled nursing communities. You’ll deliver proactive, relationship-based care directly to residents, with autonomy and support from physicians and operational leaders.

Travel to facilities is required, and you’ll work within Curana’s innovative care model that leverages technology and data analytics to improve outcomes and reduce hospitalizations.

Qualifications

• Valid NP or PA license in state of operation and current board certification.
• Master's degree in nursing or PA studies.
• Willingness to travel to multiple facilities.

Responsibilities

• Perform comprehensive health assessments and develop treatment plans for residents across communities.
• Diagnose and manage acute and chronic conditions with focus on prevention.
• Provide ongoing care management to improve quality of life and independence.
• Collaborate with physicians, staff, and families to coordinate care plans.
• Maintain timely EMR documentation and regulatory compliance.
