Job summary

Curana Health is seeking Nurse Practitioners and Physician Assistants to lead primary care for seniors across partner communities. You’ll deliver high-quality, relationship-based care directly in skilled nursing, assisted living, and independent living settings, with autonomy and support from a national clinical team.

You’ll build meaningful patient relationships, improve outcomes, and reduce unnecessary hospitalizations while working with advanced technology and a mission‑driven culture.

Qualifications

• Must hold a valid NP or PA license in the state of operation.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Willingness to travel to multiple facilities.

Responsibilities

• Deliver comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build meaningful patient relationships with residents, families, and care teams.
• Collaborate closely with physicians, clinical staff, facility leadership, specialists, and family members to coordinate care plans.
• Monitor changes in health status and intervene to prevent complications and hospitalizations.
• Maintain accurate and timely documentation within the EMR and ensure compliance with regulatory standards.
