Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver primary care in senior living facilities. You will perform direct patient care, manage chronic conditions, promote wellness, and coordinate with other providers.

This role involves travel between facilities; you will work with multidisciplinary teams, educate residents and families, and maintain meticulous EMR documentation to support high-quality outcomes.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills.
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Provide direct patient care and assessments for residents.
• Perform routine rounds and monitor health status.
• Coordinate care with facility staff and other providers.
• Educate residents and families on disease prevention and wellness.
• Maintain EMR documentation compliant with regulatory standards.
• Participate in quality assurance initiatives to improve care.
