Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in senior living facilities. You will perform comprehensive assessments, manage chronic conditions, and coordinate care with other clinicians to promote wellness for residents.

This full-time role requires travel to assigned facilities, autonomy in practice, and participation in a supportive team with strong administrative backing and modern clinical tools.

Qualifications

• Must hold valid NP or PA license in state of operation and current board certification.
• Master's Degree in Nursing or PA Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform comprehensive assessments and develop treatment plans, focusing on preventive care and chronic disease management.
• Conduct regular rounds to assess residents’ conditions and adjust plans as needed.
• Collaborate with facility staff, families, and other healthcare providers for coordinated care.
• Educate residents and families on disease prevention and wellness to support outcomes.
• Maintain accurate documentation in EMR and ensure regulatory compliance.
• Engage in quality improvement initiatives to enhance care and reduce hospital readmissions.
