Qualifications

• Masters degree in nursing or PA studies.
• Active NP/PA licensure in the state of operation.
• Strong leadership and communication skills.
• Experience delivering primary care in senior living or similar settings.

Responsibilities

• Deliver high-quality primary care in skilled nursing, assisted living, and independent living communities.
• Diagnose and manage acute and chronic conditions with focus on preventive care.
• Build strong patient relationships and conduct facility rounds.
• Collaborate with physicians, care teams, and families to coordinate care.
• Maintain EMR documentation and ensure regulatory compliance.
