Job summary

Curana Health seeks nurse practitioners and physician assistants to deliver proactive primary care in senior living settings. You will assess residents, develop treatment plans, manage chronic conditions, and partner with physicians and care teams to improve outcomes and reduce hospitalizations.

You’ll practice with autonomy in a mission-driven organization that uses advanced technology and data-driven care, with travel to assigned facilities as needed.

Qualifications

• Must hold an active NP or PA license in the state of operation.
• Master's degree in nursing or physician assistant studies.
• Strong leadership, communication, and collaboration skills.

Responsibilities

• Deliver comprehensive health assessments and develop individualized care plans.
• Diagnose, treat, and manage acute and chronic conditions with a focus on preventive care.
• Maintain EMR documentation and ensure regulatory compliance.
• Collaborate with physicians, facility leadership, and care teams to coordinate care.
