The role requires state licensure, board certification, and a commitment to senior-focused, relationship-based care with travel to multiple facilities as needed.

Qualifications

• Active NP or PA license in the state of operation.
• Board certification in your field preferred.
• Strong leadership and communication skills.

Responsibilities

• Deliver high-quality primary care to residents across communities.
• Diagnose, treat, and manage acute/chronic conditions with preventive focus.
• Maintain comprehensive EMR documentation and regulatory compliance.
• Collaborate with physicians, staff, and families to coordinate care.
• Travel to assigned facilities as needed.
