Job summary

ComplexCare Solutions is seeking a Nurse Practitioner to provide per diem in-home assessments for members in Lancaster County, Nebraska. You will spend up to an hour with each member to capture a comprehensive view of their health needs and determine care management opportunities to keep them safe at home.

Applicants must hold an active NP license with ANCC or AANP certification and CPR. Familiarity with CMS regulations, ICD-10/CPT-4 coding, and EHRs is preferred.

Qualifications

• Active un-encumbered nurse practitioner license and board certification.
• Knowledge of CMS regulations and NCQA HEDIS guidelines.
• Familiarity with ICD-10 and CPT-4 coding practices.
• Ability to conduct thorough member assessments and document findings.
• Proficiency with electronic health records and secure messaging tools.

Responsibilities

• Perform comprehensive in-home assessments, spending up to one hour per member.
• Capture health status, gaps in care and care management opportunities.
• Document assessment results and communicate findings to PCPs and health plans.
• Provide evidence-based treatment planning and appropriate referrals.
• Maintain HIPAA compliance and complete required encounter documentation.
