Job summary

Your Health in Chapin, SC is seeking a qualified Nurse Practitioner to serve patients in a Chapin area clinic. As a Provider, you will evaluate high-risk patients, develop care plans, coordinate with an interdisciplinary team, and support care management to improve outcomes.

This is a full-time, salary-based role with the opportunity for a monthly bonus. Shifts include 12-hour (7 days on/7 off) or 8-hour (Mon-Fri, 8:00am-5:00pm). Travel between offices is expected where needed.

Qualifications

• Licensed NP/PA/MD in state of employment.
• Experience in home health, clinic, facility, telehealth, or specialty care.
• Strong communication and collaboration within an interdisciplinary team.
• Ability to evaluate high-risk patients and coordinate care plans.
• Commitment to compassionate, patient-centered care and excellence.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity and outcomes.
• Communicate with case managers, coordinators, social workers, and pharmacists to manage patient care.
• Participate in team meetings to discuss progress and care coordination.
• Document care activities accurately to support care plans and compliance.
• Travel between offices, facilities, etc., and work flexible hours.
