Job summary

Your Health Organization seeks a compassionate Nurse Practitioner to serve patients in the Chapin area. The Provider will evaluate high-risk patients, coordinate care plans, and support care management across clinics, homes, and telehealth settings.

This is a full-time salary-based position with the opportunity to earn a monthly bonus and work with an interdisciplinary care team to improve patient outcomes.

Qualifications

• Nurse Practitioner, PA, or Physician licensed to practice in the state of employment in good standing.
• Experience delivering patient care in home health, clinic, facility, telehealth, or specialty care settings preferred.
• Strong communication, collaboration, and interpersonal skills for interdisciplinary teams.
• Ability to evaluate high-risk patients, develop individualized care plans, and coordinate care activities.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team members to ensure continuity of care.
• Communicate regularly with case managers, coordinators, social workers, and pharmacists to coordinate patient care.
• Participate in interdisciplinary team meetings to discuss patient progress and treatment plans.
• Document care activities accurately to support patient care plans and compliance.
• Provide patient education and resources related to conditions, treatment plans, and self-management.

We are seeking qualified Nurse Practitioner to service patients in the Chapin area. As a Provider, you play a critical role in delivering exceptional patient care in a clinic setting. You are an integral part of the care team, collaborating with multiple healthcare professionals to ensure comprehensive and effective patient management. Your responsibilities include evaluating high-risk patients, coordinating care plans, and providing care management to improve patient outcomes.

Our Provider role performs visits in Chapin clinic daily. This is a full-time salary-based position, with the opportunity to receive a monthly bonus.

The following shift(s) are available:

• 12-hour shift (working 7 days on 7 days off)
• 8-hour shift (Monday-Friday; 8:00am-5:00pm)

What Are We Looking For?

Your Health is currently looking for a Nurse Practitioner to join our growing primary care family. A successful Provider will be able to perform these essential duties and responsibilities. Areas of Responsibility:

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
• Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members to coordinate patient care.
• Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
• Foster effective communication and collaboration to facilitate seamless patient management and interdisciplinary teamwork.
• Meet staff efficiency expectations while providing high-quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.
• Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
• Provide patients with education and resources related to their conditions, treatment plans, self-management strategies, and preventive care measures.
• Offer support and guidance to patients and their families to enhance their understanding of healthcare options and promote active participation in their care.
• Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
• Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed

Qualifications:

• Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
• Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
• Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.
• Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.
• Commitment to delivering compassionate, patient-centered care and promoting a culture of excellence in healthcare delivery.
• Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
• Ability to read and communicate effectively.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to manage and demonstrate effective leadership skills.
• Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
• Ability to foster a cooperative work environment.
• Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
• Must be able to work flexible hours and travel between offices, facilities, etc. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
