Job summary

Your Health Organization in Chapin, SC is seeking a Nurse Practitioner to serve patients in our clinic network. You will evaluate high-risk patients, develop individualized care plans, and coordinate care with an interdisciplinary team to improve outcomes and ensure comprehensive, patient-centered care.

This full-time, salary-based role offers a monthly bonus opportunity and requires flexibility to cover 12-hour or 8-hour shifts, with travel between offices as needed.

Qualifications

• Licensed NP/PA/MD in state of employment.
• Experience in home health, clinic, facility, telehealth, or specialty care.
• Strong communication and collaboration skills.
• Ability to develop care plans and coordinate care activities.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care with interdisciplinary team to ensure continuity and outcomes.
• Communicate with case managers, clinicians, pharmacists, and caregivers.
• Participate in team meetings to discuss progress and care coordination.
• Document care activities and ensure regulatory compliance (HIPAA/Medicare).
• Provide patient education and resources on conditions and self-management.
• Travel between offices and facilities as needed.
• Maintain patient-centered care and quality outcomes.
