Job summary

Your Health Organization seeks a compassionate Nurse Practitioner to serve patients in the Chapin area. The Provider will evaluate high-risk patients, coordinate care plans, and support care management across clinics, homes, and telehealth settings.

This is a full-time salary-based position with the opportunity to earn a monthly bonus and work with an interdisciplinary care team to improve patient outcomes.

Qualifications

• Nurse Practitioner, PA, or Physician licensed to practice in the state of employment in good standing.
• Experience delivering patient care in home health, clinic, facility, telehealth, or specialty care settings preferred.
• Strong communication, collaboration, and interpersonal skills for interdisciplinary teams.
• Ability to evaluate high-risk patients, develop individualized care plans, and coordinate care activities.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team members to ensure continuity of care.
• Communicate regularly with case managers, coordinators, social workers, and pharmacists to coordinate patient care.
• Participate in interdisciplinary team meetings to discuss patient progress and treatment plans.
• Document care activities accurately to support patient care plans and compliance.
• Provide patient education and resources related to conditions, treatment plans, and self-management.
