Job summary

Mannacare Health and Wellness Medical Center in Lima, Ohio is seeking a licensed Nurse Practitioner to join our primary care team.

You will provide comprehensive primary care services to patients across the lifespan, including exams, chronic disease management, preventive care, and diagnosis/treatment of acute conditions, working closely with our clinical and administrative teams to deliver consistent, high-quality patient care.

Qualifications

• Active Ohio APRN license with Family Nurse Practitioner (FNP) certification.
• Active Ohio APRN license with Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) certification.
• Active Ohio APRN license with Adult Nurse Practitioner (ANP) certification.
• Active Ohio Physician Assistant (PA-C) license with primary care experience.
• Active DEA registration or ability to obtain one.
• Experience with Tebra or comparable EHR/practice management software preferred.
• Experience in a primary care or family medicine setting preferred.
• Strong communication skills and comfort working within a collaborative, multi-provider practice.
