Job summary

ComplexCare Solutions is seeking a Nurse Practitioner (Per Diem) to conduct home and virtual assessments nationwide with a focus on Lane County, OR. Position includes a generous sign-on bonus and a pay range reflecting variable assessments and locations.

Responsibilities include comprehensive reviews, documentation, and care coordination with PCPs and health plans. In-home visits, telehealth options, and adherence to CMS/NCQA guidelines are required.

Qualifications

• Active unencumbered nursing license.
• ANCC or AANP board certification as a Nurse Practitioner.
• Current CPR certification.
• Vaccination and preventive testing as required prior to hire.

Responsibilities

• Review member data and document assessment findings.
• Maintain CMS/DHHS compliance and proper coding.
• Coordinate with PCPs and health plans for follow-up.
• Provide evidence-based treatment plans for chronic conditions.
