Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking an experienced Nurse Practitioner or Physician Assistant to provide primary and episodic care in skilled nursing facilities. You will manage your own panel and build long-term relationships with residents and families.

Work six-hour shifts with morning rounds and no on-call or overnights. You can chart from home after rounds, and earnings are production-based, with potential to earn up to $624/day.

Qualifications

• Board-certified NP (AGNP/FNP) or PA-C with active, unrestricted license.
• Unrestricted NP or PA license in the operating state.
• Master's degree in Nursing (NP) or from an accredited PA program.

Responsibilities

• See roughly 16–20 patients per day, including wellness visits, H&Ps, and chronic care management.
• Coordinate with physicians, specialists, and facility staff to ensure seamless care.
• Communicate with families and participate in care conferences as needed.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–20 patients a day — a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.

Qualifications

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation.
• Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program — required for the corresponding credential.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.
