Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking an experienced Nurse Practitioner or Physician Assistant to provide primary and episodic care in skilled nursing facilities.

You will manage your own panel and build lasting relationships with patients and families, with autonomy and a flexible schedule. Shifts are 6 hours, typically mornings; charting from home is supported, and there is no on-call.

Qualifications

• Board certification required (AGNP, FNP or PA-C) — active and unrestricted.
• Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation.
• Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program — required for the credential.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients daily, including wellness visits and acute issues.
• Coordinate with collaborating physicians, specialists, and facility staff to keep care seamless.
• Stay in touch with families and join care conferences where appropriate.

Nurse Practitioner or Physician Assistant — Skilled Nursing Facility

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day
