Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) seeks an experienced Nurse Practitioner or Physician Assistant for a Skilled Nursing Facility, full time. This independent role builds your own panel and long-term relationships with residents and families.

You will see roughly 16–20 patients daily, handling chronic and acute issues, wellness visits, and care coordination. 6-hour shifts with no on-call or overnights, chart from home where possible.

Qualifications

• Board certification (AGNP/FNP for NP; PA-C for PA) active and unrestricted.
• Unrestricted state license to practice as NP or PA.
• Master's degree in Nursing or equivalent PA credential required.
• Skilled nursing or long-term care experience preferred.
• Strong independent clinical judgment and fast pace adaptability.

Responsibilities

• See 16–20 patients per day with a mix of chronic and acute issues.
• Conduct annual wellness visits and H&Ps; review meds and orders.
• Coordinate with collaborating physicians, specialists and facility staff.
• Stay in touch with families and join care conferences when appropriate.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–20 patients a day — a mix of long-term chronic care and acute issues. Qualifications

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation.
• Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program — required for the corresponding credential.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.
