Job summary

PruittHealth Hospice is seeking a Nurse Practitioner to join our hospice services, providing compassionate care in patients' homes or care facilities. This full-time role emphasizes top-tier pay, comprehensive health plans, and professional development.

The NP will perform holistic hospice assessments, manage care plans, document thoroughly, and adhere to policies while collaborating with an interdisciplinary team to ensure high-quality patient care and continuity.

Qualifications

• Current, active, and unrestricted RN license in state of practice.
• Current, active, and unrestricted NP license in state of practice.
• Board-certified by AANP or ANCC in adult, geriatric, or family/general primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
