Job summary

PruittHealth Hospice in Georgia seeks a Nurse Practitioner for PRN hospice services. The role involves home or facility visits, comprehensive assessments, and management of care plans for adults and geriatrics. You will document thoroughly and follow established policies to ensure continuity of patient care.

Eligible candidates hold NP certification (AANP/ANCC) and RN licensure, preferably with hospice experience. Competitive pay and benefits are provided against a flexible schedule.

Qualifications

• Current, active, and unrestricted NP licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients; evaluates and manages plan of care.
• Complete all documentation and records accurately and timely; follow policies and procedures.
