Job summary

PruittHealth Hospice is seeking a Nurse Practitioner (NP) to deliver hospice and palliative care services in patients' homes or care facilities. PRN or part-time options are available to fit your schedule.

The NP will perform adult/geriatric assessments, develop and manage care plans, document thoroughly, and collaborate with an interdisciplinary team to ensure compassionate, high-quality patient care. Board certification and state licensure are required.

Qualifications

• Current, active, and unrestricted Registered Nurse (RN) and Nurse Practioner (NP) licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
