Job summary

PruittHealth Hospice in North Carolina is seeking a Nurse Practitioner for full-time, first-shift work delivering compassionate hospice care in home or facility settings. Responsibilities include comprehensive geriatric assessments, care planning, thorough documentation, and teamwork with clinicians to maintain continuity of care.

• Current, active, and unrestricted RN licensure in state of practice.
• Current, active, and unrestricted NP licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
