Job summary

PruittHealth Hospice is seeking a Nurse Practitioner for hospice services, offering full-time employment. The role focuses on comprehensive patient assessments, plan-of-care management, and seamless documentation within a hospice setting, whether in patients' homes or care facilities.

Qualifications

• Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
• Commitment to caring for patients and partners
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
