Job summary

PruittHealth Hospice is seeking a PRN Nurse Practitioner (NP) to provide adult and geriatric hospice care in home or care facility settings. You will assess patients, develop and manage care plans, document thoroughly, and collaborate with an interdisciplinary team.

Qualifications

• Active RN license in practice state.
• NP licensure with board certification in geriatric or primary care.
• Experience in hospice or palliative care preferred.

Responsibilities

• Perform comprehensive adult/geriatric hospice assessments and plan of care.
• Collaborate with interdisciplinary team to coordinate care.
• Complete timely documentation and maintain patient records.
• Adhere to scope of practice and organizational policies.
• Maintain continuity of patient care in hospice settings.

Qualifications

ICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

• Current, active, and unrestricted Registered Nurse (RN) and Nurse Practioner (NP) licensure in state of practice
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care
