Job summary

PruittHealth Hospice is seeking a Registered Nurse (RN) Case Manager to provide exceptional hospice care in patients' homes or facilities in South Carolina. This full-time role may include PRN shifts, with a focus on compassionate, patient-centered services and comprehensive documentation.

Qualifications

• Must hold current, active RN licensure in the state of practice (SC).
• Licensure must be unrestricted and in good standing.

Responsibilities

• Complete evaluations, assessments and treatments per individualized care plan.
• Document and maintain records timely and accurately.
• Follow policies and procedures related to hospice scope of practice.
• Maintain continuity of patient care in home or facility hospice settings.
