Curana Health is hiring Nurse Practitioners and Physician Assistants to lead primary care teams in senior living communities and skilled nursing facilities. You will diagnose, treat, and manage residents with a focus on preventive care, while building strong patient relationships and delivering high-quality care across settings.

You’ll practice with greater autonomy, supported by experienced physicians, operational leaders, and advanced technology designed to improve outcomes.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Deliver High-Quality Primary Care: perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build Meaningful Patient Relationships: conduct regular facility rounds and develop strong relationships with residents, families, and care teams.
• Monitor changes in health status and intervene to prevent complications and hospitalizations.
• Educate residents and caregivers on disease management, preventative care, and wellness strategies.
• Collaborate Across the Care Continuum: coordinate care plans with physicians, staff, leadership, specialists, and families.
• Serve as a trusted clinical resource and contribute to interdisciplinary care discussions.
